Use this form to apply for or renew a Blue Badge. You can also use it to replace a lost, stolen or damaged badge.
You must have a My account to apply for a Blue Badge. If you are not registered, you must do this first.
You will need to include the following evidence:
- proof of identity such as a birth certificate, passport or driving licence - photograph or scan copy
- proof of address such as a council tax bill or government letter - photograph or scan copy
- a recent digital photo showing your head and shoulders
- your National Insurance number if you have one
- the details of your current Blue Badge if you’re reapplying
You will also need to provide medical or other evidence to support your reason for application.
Some reasons need specific evidence, which includes:
- terminal illness - SR1 form
- PIP mobility (moving around) - DWP PIP award letter dated within last 12 months
- PIP mobility (planning and following journeys) - DWP PIP award letter dated within last 12 months
- Disability Living Allowance (higher rate for mobility) - DWP DLA award letter dated within last 12 months
- blind or severely sight impaired - certificate of visual impairment
- War Pensioners Mobility Supplement - WPMS letter from Veterans UK dated within last 12 months
For the Armed Forces Compensation Scheme, you will need both:
- AFCS letter from Veterans UK confirming tariffs 1 to 8
- certificate confirming disability from Veterans UK
