Use this form to make an Early Help referral. Read the following criteria before making this type of referral.

Early Help request for support

You can request support for children with additional needs.

Parents seeking behavioural, emotional or mental health support for their child should use this referral form to request help.

Some children may also be vulnerable and showing early signs of abuse and, or neglect. Their needs may not be clear, not known or not being met. The parents of some children who require early help may not have prioritised their children’s needs and, or have limited parenting capacity.

This is the threshold for a multi-agency early help assessment to start.

These are children who require a lead professional for a co‐ordinated approach to the provision of additional services such as family support services, parenting programmes and children’s centres or family hubs. These will be provided by universal or targeted services working with others but does not include services provided by Children’s Social Care.
