Housing and Council Tax Benefit - Consent for Third Party Disclosure

The information that you give us in support of your application for Housing and Council Tax Benefit is confidential. So, the law requires us to confirm the identity of a benefit applicant whenever a request for information is made.

Sometimes it can be difficult for people to deal with their own claim, but as the law is very strict about the disclosure of information, we will need written permission before we can give any information to someone acting on behalf of an applicant.

If you want someone else to deal with your claim, you can use our Consent for Third Party Disclosure form. You can fill this in online, but you will need to print it off once you have finished, sign it and then post it to the address given on the printed form.

This form will provide us with your consent to disclose information that is protected under the provisions of the Data Protection Act 1998 and gives your permission for Bracknell Forest Borough Council Benefits Service to speak to the person named on the form about your benefit claim.

Unless you state otherwise, we will assume that you give your consent to disclose details of any matter relating to your claim to the person named on the form.
