Adult Social Care
Welcome to Bracknell Forest’s Adult Social Care and Health Department.
Our approach is to enable personalised care and support arrangements. Each and every one of us is unique: our experiences, our interests, our friends, our families, our hopes and our fears. It therefore follows that the supports a person needs and to continue to live as independently as possible should be equally as individual. This principle is fundamental to the way on which we work with people and their families.
We offer support for people to determine what support they need, and how this is best arranged to suit their lifestyle. If people are not eligible for adult social care, we can give them information about other ways they could get help, and organisations that can assist. We have developed web-based information iHub and have also written a guide to how people can help to maintain their independence for as long as possible. Prevention Guide
The focus of our support is to enable people to retain their independence, which will mean people can stay in their own homes for as long as possible.
Support may be needed for a ‘crisis’ or a longer period, depending on the circumstances.
We support people who fall into the following groups:
- Older people
- People with a learning disability
- Asylum seekers
- Carers
- People affected by HIV/Aids
- People with mental health needs
- People who misuse substances
- People with long term conditions
- People with an Autistic Spectrum Disorder (ASD)
