Glenfield House

Glenfield House is a supported living project for people recovering from mental health issues.
Tenants hold a 2 year tenancy with Bracknell Forest Homes and are allocated a self contained furnished flat, with a bathroom, kitchen and open plan lounge/bedroom, with a link to forest care.
Glenfield House has large communal areas for tenants to use, currently we have an IT room, training room, large TV lounge and a lovely garden with seating areas, we also have allocated parking for tenants.
Glenfield House has 2 respite flats and 2 homeless beds which have a kitchen, bathroom, open plan lounge/bedroom and shared use of the rest of the building. The respite flats are usually used for one week at a time with referrals being made by the community mental health team.
The Glenfield House team consists of 1 Supported Living Manager, 6 Supported Living Officers and 1 Admin Assistant.
Referrals to the Glenfield Supported Living Project are made by care managers from the Community Mental Health Team. All referrals come with an up to date risk assessment, upon receipt of the referral Glenfield staff will arrange an assessment to see if the project is suitable to meet the person’s needs.
All tenants at Glenfield are allocated a keyworker who will work with the tenant in achieving the goals set out in their support plan; the support plan is then reviewed on a frequent basis.
The goals in the support plan are all led by the tenant and the keyworker will support the tenant in working towards their goals with an overall aim of moving on from Glenfield House to living on their own in the community, although support can still be given to people who make this move by our outreach service.
Staffing at Glenfield is provided from 8am to 9.30 pm Monday –Sunday.
Glenfield Supported Living Outreach Service
Glenfield Supported Living Outreach Service provides support to people living in the community who are recovering from mental health issues.
Referrals are made via the community mental health team and include an up to date risk assessment, once the referral is received a member of the Glenfield outreach team will visit the person and complete an assessment of suitability to the service. Once the assessment has taken place a support plan is drawn up between the individual and the outreach keyworker giving clear identified targets for the individual to work towards, this is reviewed on a frequent basis.
The support given can range from budget management support, support in daily living skills to support in using public transport and accessing college course
Support for the outreach service can be given by phone support, one to one keyworking or group work with other people accessing the outreach service.
