Bracknell Forest Council support people with dementia and long term conditions based within the Community Support and Wellbeing Team.
We offer a flexible outcome based service to assist people to remain in their own home.
We also provide services through contracts with approved providers in the independent sector or directly provided services.
You would need to make contact with our Community Response and Re-ablement Team for an assessment of your needs.
The Dementia Team and the Long Term Conditions Team both regularly consult with people who use their service and other professionals which form part of the Care Quality Commission key inspection. The inspector looked at how well the service was meeting the standards set by the Government. The Business Support Team monitor and review our policies and procedures to ensure consistent service quality.
