Home Care
Do you need help in your home to keep you independent?
If you are a vulnerable person or have a disability, Bracknell Forest may be able to help you to stay in your home by providing personal, practical, social care and support in your own home.
The Home Care Service provides a range of support to vulnerable people to assist with maintaining their independence in their own home.
You can ask for an assessment to see if you qualify for these services by contacting us using the details on the right.
The assessment is free of charge but you may have to pay for some of the services we provide.
Intermediate Care
Bracknell Forest's Intermediate Care Service is a multi-disciplinary team of:
- Occupational Therapist
- Physiotherapist
- Intermediate Care Assistants
- Home Support Manager
- Care Management Support
and it provides:
- a rapid response service in times of health crisis
- intensive short term support in the home
- rehabilitation in the home or community settings
- support following discharge from hospital
The programme will last for a minimum of one assessment to a maximum of 6 weeks where active therapy is required.
The service is accessed by:
- your GP
- your hospital consultant
- care management via the Social Services Access Team (details above).
