Support for adults with learning disabilities

At Bracknell Forest, we have a special team that looks after people with learning disabilities.

This team is called the Community Team for People with Learning Disabilities (CTPLD).

The CTPLD team can help you with lots of things, like:

  • advocacy (someone who can help you say what you want)
  • housing
  • help for carers
  • friends and relationships
  • learning new skills
  • keeping healthy
  • travel
  • being and feeling safe
  • learning
  • money and benefits
  • work

Social care team

The social care team will:

  • support you to identify the help you need
  • talk to you about what you want in your life and how you want to live it
  • create a plan together with you
  • help you to feel and stay safe

Challenging behaviour specialist

The challenging behaviour specialist will:

  • help you and other people understand your behaviour
  • help you manage your behaviour

Psychiatrist

The psychiatrist will:

  • talk with you and give you advice about your mental health
  • make sure that you are taking the right tablets

Learning disability nurse

The learning disability nurse will:

  • check your health
  • help you get the health care that you need
  • talk to you about how to stay healthy
  • help you if you need to go to hospital or health appointments

Psychologist

The psychologist will:

  • help you understand about how you feel
  • talk to you alone and in private
  • help you to know about your behaviour

Physiotherapist

The physiotherapist will:

  • help you keep active and moving
  • help you go to clinics if you need to

Dietitian

The dietitian will:

  • talk to you about diet and eating healthy food
  • help you with planning your meals

Speech and language therapist

The speech and language therapist will:

  • help you with your communication
  • help you if you cough when you eat and drink

Occupational therapist

The occupational therapist will:

  • help you learn new skills
  • help you work in groups
  • give advice about equipment to help you

Health support worker

The health support worker will:

  • help to keep you healthy
  • help you to get to health appointments at the hospital or doctor

If you are not happy with what someone has said, you can meet someone else. Tell us!

Who can get an assessment

You can get an assessment if you are:

  • 18 years old or nearly 18 years old
  • have a learning disability
  • live in Bracknell

You can contact us and we will send someone out to speak to you. Friends and family can help you say what you need or we can find you an advocate (someone who can help you say what you want).
