Care coordinators

If you have a diagnosis of dementia, or care for a person with a diagnosis, you should have access to a care co-ordinator. This person will help you find and manage support.

A care co-ordinator could be a:

  • dementia adviser
  • care manager
  • social worker
  • mental health nurse
  • another practitioner involved in your care

Depending on your needs, you could be offered support including information and advice and signposting to other services. You may also get help with assessments:
