This form is for use by registered skip operators to apply to extend a current permit to place a builder's skip on a public road or highway.
The cost is £16 per week or part thereof. An extension to a permit cannot be granted without payment.

You will need to give us:
- skip operator name and licence number
- reference number of the permit you want to extend
- start and end date of the extension period
- payment details
