At the appointment

You have married the father/second parent since the birth

Since the birth, if you have married the father/second parent outside of the UK, you must first get authorisation for re-register from the General Register Office.

Re-registration application forms can be obtained from any register office in England or Wales or from GOV.UK:

At the appointment you will need to provide the completed re-registration form and show your original marriage certificate. Please note that the application form normally needs to be signed by both parents, but only one parent needs to attend the appointment.

    At the appointment you will need to provide a completed re-registration form signed by both parents.

    If you are adding the father's details and you are not married, you both need to attend the appointment. Otherwise, you can do this by statutory declaration.

    If you want to do this by statutory declaration, you must first get authorisation for re-register from the General Register Office.
