Payment is required after you apply

After you have submitted your application, you need to come back to this page and pay for your application.

Log onto the Disclosure and Barring Service (DBS) system

First, log onto the DBS system. You may want to right click and open this in a new tab so that you can keep referring to this guidance while you are filling it in.

Log onto the DBS system

You may be taken straight to a new application. If not, select the box called "Standard / Enhanced DBS application".

Your login details are case sensitive.

Organisation reference

For a voluntary licence enter the organisation reference: CYPLVOLCHAP

For a professional licence enter the organisation reference: CYPLPROCHAP

Organisation code

Leave the organisation code blank then select enter. 

Statement of fair processing

You will now see the ‘CRVS Statement of Fair Processing’ outlining our terms and conditions.

Read the statement and tick the box to confirm you have read, agreed and understood this statement.

Once you have ticked the consent box select ‘next’.

Application pre-entry statement

You will now see the application pre-entry statement. This gives information about the requirements by the DBS about acceptable identification in support of your application.

Tick the consent box to confirm you have read and understood this statement and select ‘next’.

Take care filling in each section of the application form and complete all fields. Mistakes made in the application form will cause delays. 

Section 1 – about you

Enter your personal details. If you have any middle names make sure that you enter these details, particularly those that appear on your identity documents. Any missing information from this or any other section can delay the processing of your DBS application.

Once you have completed this section select ‘next’.

Section 2 – address history

Enter your current address. If you have not lived at this property for more than 5 years, you will be taken to another screen. Here you need to add any extra addresses so that your address history goes back 5 years. 

Address dates

There cannot be any gaps or overlaps in your address history. Make sure that the month and year of each address follows that of the previous address. Students who switch between their permanent residence and education establishments must enter each address with dates consecutively. For example:

  • parents' address - December 2009 to January 2010
  • university address - January 2010 to March 2010
  • parents' address - March 2010 to April 2010

Do not run address dates simultaneously as your application form will be rejected by the DBS. Once an address has been entered, you can edit the information by selecting the ‘edit’ button.

Postcode

You must include a full postcode for all UK addresses. If you cannot remember the postcode you can use the Royal Mail postcode finder

No fixed abode UK

If you were of no fixed abode within the UK, enter the nearest hostel address to the location you were based.

Foreign addresses and travelling overseas

If you have travelled overseas and cannot supply the address for a foreign country then enter ‘no fixed abode’ for Address Line 1. Then enter the town or city and the country in which you were living. 

Once you have completed this section select ‘next’.

Section 3 – additional info

Place of birth

Enter details of your place of birth.

Nationality at birth

Enter your nationality at birth and state yes or no as to whether this has changed since you were born. If this has changed, you then need to enter your current nationality.

Changed surname

If you have changed your surname, provide your birth surname and the year you changed it. Make sure that where names change they run in date order and with no gaps.

Other names

Provide details of any other names used during your lifetime and the dates during which the names were used. Enter each first name, middle name and last name separately using the 'add name' button.

Also include details if you have used any aliases, for example, William known as Bill.

Section 4 – employment

Position applied for

Enter the correct job role: voluntary chaperone or professional chaperone.

Do not use any abbreviations and do not enter just volunteer or professional or chaperone. 

Failure to enter the correct job role may cause your application to be delayed.

Employer name

Leave this as it is. This is defaulted to the organisation you are applying through.

Conviction history

If you have never been convicted of a criminal offence select ‘no’.

If you have been convicted of a criminal offence, or received a caution, reprimand or warning that is ‘protected’ as defined by the Rehabilitation of Offenders Act, select ‘no’.

If you have ever been convicted of a criminal offence, caution, reprimand or warning that is not ‘protected’ as defined by the Rehabilitation of Offenders Act, select ‘yes’.

For exceptions to this legislation or for more information, read the Rehabilitation of Offenders Act 1974.

Once you have completed this section select ‘next’.

Section 5 – confirm

You must now read through your application to make sure there are no omissions or mistakes. If you want to change a section of your application select ‘edit’ next to the section of the form that you want to change. At the end of the application make sure that you complete the 'Applicant Consent' section.

Now select ‘complete’.

You have now completed your application form and will be given a submission reference number. Make a note of this number for the next step of your online application. If you entered an email address in your application, you will receive a confirmation email which will include your reference.

Pay for your application

Pay online for your application. Make sure you enter your DBS submission reference number in the application reference box and select either:

  • Volunteer DBS check £10
  • Enhanced DBS check £50 (for a professional licence)

The last question is about external ID validation. Select 'no'.
