The Mandatory Disabled Facilities Grant is subject to eligibility criteria. Anyone who does not meet the criteria will not be eligible for Grant funding.
You can review the eligibility criteria, application management options, application and works stages, by visiting sections 1 to 7 of these advice and guidance pages. Make sure you familiarise yourself with these sections of the advice and guidance before you fill in an application form.
Prior to filling in your application for funding, it is important to make sure you have to hand all relevant documentation, supporting evidence, and contact details for all relevant parties.

All applicants are required to provide proof of identity and proof of address.
Adult application
If the applicant (the owner or tenant of the property) and disabled person (the relevant person) are 2 different people, then this information must be provided for both the applicant and the relevant person.
Proof of identity
You will need to upload 2 of the following.
- provisional or full driving licence
- passport
- birth certificate
- marriage certificate
Proof of address
You will need to upload 3 of the following. These must be dated within the last 6 months.
- bank statement (must show address)
- pension letter
- utilities bill
- Council Tax bill
- benefit confirmation letter
- child benefit or child tax credit letter
Child application
If you are making an application for funding to meet the needs of a child, you will need 1 proof of identity and 1 proof of address for the child. The acceptable documents are listed below.
You must be the tenant or the owner to make this application and, as well as the documents needed for the child, you must provide the documents for yourself, as listed above under Adult application.
Proof of child identity
You will need to upload 1 of the following.
- Birth Certificate
- Passport
Proof of child address
You will need to upload 1 of the following.
- Child Benefit Award Letter - you can get proof of child benefit on GOV.UK
- Child Tax credit letter
Passported application - applicants in receipt of a passported benefits
Some applications will be passported. This means that you will not have to pay for works up to the value of £30,000.
Whether your application is passported, will depend on:
- your age
- if you receive a qualifying passporting benefit
Adaptations for people under the age of 18 years are automatically passported.
If you are applying for funding and receive 1 of the following passported benefits, you will be required to provide proof of entitlement, such as your award letter.
- Universal Credit
- Income-related employment and support allowances (ESA)
- Income support
- Income-based jobseeker’s allowance
- Guarantee pension credit
- Housing benefit
- Working tax credit or child tax credit (where annual income for the purposes of assessing tax credit entitlement is less than £15,050)
Test of Resources (TOR) - applicants not in receipt of a passported benefit
If you do not receive a passported benefit listed above, you will need to provide supporting financial information so we can carry out a means test.
The means test helps us calculate what contribution, if any, you will need to make towards the cost of works.
If you have not already done so, use the self-assessment calculator to get an estimate of any contribution you may need to make towards the cost of work. You do not need your supporting information to use the calculator. After receiving the report it generates, you may decide not to continue with making an application. If you are unsure what the calculations in the report mean, contact the disabled facilities grant service who will be happy to explain these to you. This is a simplified calculator and outcomes are estimates only.
If you are continuing with your application, make sure the following supporting information is provided for the disabled person and their partner, if applicable.
Failure to provide the required information will delay your application being assessed. All financial supporting information must be dated within the last 3 months.
- bank statements from your current account or accounts fully covering the last 3 full months
- proof of any household income over last 3 months
- evidence of savings in your bank, building society or post office account or accounts
- evidence of savings bonds or certificates including premium bonds, share certificates
- other capital and investments including property
- your last annual certified accounts, if self-employed
Owner Occupier
If you own the property together with others, it is advisable to have all owners present when filling out the application to make sure everyone agrees and consents to any applicable Land charges.
If you own a qualifying houseboat or caravan, you will need to provide proof of ownership.
Tenant
You will need to provide the contact details for your landlord, including telephone number and email address. Make sure you have these ready before you fill in the application form.
