Approaching adulthood team

The Approaching adulthood team is a specialist team who identify and support young people from the age of 14 who may have adult social care needs and have an EHCP (Education Health and Care plan).

We have put together the preparing for adulthood pathway guide.

This guide is for young people aged 14 to 25 with SEND who are preparing for adulthood. It will also be helpful to their parents, carers, and the professionals who support them.

It explains how services should work together to help young people with special educational needs, disabilities, learning disabilities, or mental health needs plan for adult life.

The guide may also be helpful if you have health or care needs but do not have an EHCP.

If you are in crisis or a person is at risk, please contact us by phone. 

Outside of normal working hours, the phone number will divert to the Emergency Duty Service. They will respond to anything that cannot wait until the next working day.
