Use this form to apply for free school meals for your child. You can apply for free school meals for up to 4 children using this form. If you would like to apply for more than 4, email fsm@bracknell-forest.gov.uk.
You will need to provide:
- contact details of the parent or carer receiving the benefit
- parent or carer's National Insurance number
- parent or carer's date of birth
- the name, date of birth and school attended for each child
Make sure that the details you put in the form match your universal credit statement. This includes things like name, date of birth and national insurance number.
