How we carry out assessments

Our assessment process involves 3 meetings that focus on your short-term and long-term needs. These get straight to the issues you're experiencing so we can solve them together.

Our first meeting focuses on:

  • quality, short-term early help
  • help to prevent or delay the need for further help

This help is available to you whether you are eligible for help under the Care Act or not.

Our second meeting is to help you regain control and independence. We focus on short-term interventions to help you resolve an immediate need or crisis.

Our third meeting marks the point where we establish long-term support as part of our statutory duties and processes.

We review long-term support for carers every year.

If you are in crisis or a person is at risk, contact us by phone.

Outside of normal working hours, the phone number will divert to the Emergency Duty Service. They will respond to anything that cannot wait until the next working day.
