Free NHS remote monitoring service

The Technology Integrated Health Management (TIHM) Monitoring Service uses remote digital monitoring devices installed in homes to collect information about people’s health and wellbeing. Health issues identified by the technology are followed up by a dedicated NHS Monitoring Team who provide swift advice and support.

Bracknell Community Mental Health Team for Older Adults

The Bracknell Community Mental Health Team for Older Adults (CMHTOA) team work closely with other health and social care teams as well as voluntary organisations to give older adults help after a mental health diagnosis.

Referrals to this team are from your GP or health or social care practitioner. 

Some services may be available by self referral. To find out more, please call the CMHTOA team on 01344 823220.
