Community Team for People with Learning Disabilities (CTPLD)

We are a specialist team dedicated to supporting individuals who have learning disabilities.

Who we support

Our services are available to adults with a learning disability and their carers.

We carry out assessments of need for both the individual and their carer to establish if they are eligible for support from the local authority.

Working with you

We collaborate with you and your circle of support (those who are important in your life) to help you achieve your goals and desired outcomes. We look at the following areas:

  • community participation
  • health and wellbeing
  • independent living
  • employment
  • support to keep you safe

We recognise your skills, abilities and experiences and value the support you get from family, friends and your community. We work alongside you and do not make decisions for you.

Social care support

Our social care support includes:

  • assessing eligibility based on support needs
  • person centred support planning to meet needs
  • reviews to make sure support is working and adjusting plans as needed
  • completing mental capacity assessments and best interest decisions
  • signposting or referral to relevant services

We work in a preventative way by:

  • offering early advice and guidance
  • helping you access community support and local resources
  • supporting you to stay well and independent for longer
  • working with other services to reduce risks and avoid crisis situations

Wellbeing means different things to different people. When we work with you, we think about:

  • your physical and mental health
  • your independence and ability to make choices
  • your personal relationships and social connections
  • your safety and security
  • your sense of purpose, dignity and control over your life

Our role is to listen to what matters most to you and support outcomes that are meaningful to your life, not just services.

Outside of normal working hours, the phone number will divert to the Emergency Duty Service. They will respond to anything that cannot wait until the next working day.
