Use this form to apply for a skip operator licence. The council registers operators who wish to hire out skips to be placed on the public road. The cost of the licence is £105 and you will be able to pay online at the end of the form.

- operation full address
- name of insurer
- level of cover (minimum of £5 million)
- policy number and validity dates
- public liability insurance certificate
- goods vehicle operator's licence
- waste carrier's licence certificate
- waste transfer notice
